Spiriva Drug Interactions
There are quite a number of drugs and medications that can potentially interact with Spiriva. Usually, these drugs also have anticholinergic effect that may potentiate the side effects of Spiriva. There are also some drugs that can interfere with the body’s ability to metabolize this medication. Caution should be taken when using any of these medications.
Significant Drug Interactions
Some of the drugs that are known to interact with Spiriva include the following:
Acetylcholinesterase inhibitors – These medications are often prescribed for patients diagnosed with Alzheimer’s disease or dementia. Combining acetylcholinesterase inhibitors with Spiriva can decrease the pharmacologic effects of both medications. Your healthcare provider may need to closely monitor you for possible dose adjustment.
- Donepezil (Aricept)
- Galantamine (Razadyne)
- Rivastigmine (Exelon)
- Tacrine (Cognex)
Anticholinergics – Spiriva is also known to cause anticholinergic effects. Concomitant use with other anticholinergic drugs may increase the risk of anticholinergic adverse effects, particularly constipation, difficulty passing urine, dry eyes, and dry mouth.

- Benztropine (Cogentin)
- Clidinium (Librax)
- Clozapine (Clozaril)
- Darifenacin (Enablex)
- Dicyclomine (Bentyl)
- Dimenhydrinate (Dramamine)
- Haloperidol (Haldol)
- Homatropine (Hycodan)
- Hyoscyamine (Levsin)
- Ipratropium (Atrovent)
- Methscopolamine (Pamine)
- Scopolamine (Transderm-Scop)
- Tolterodine (Detrol)
- Trospium (Sanctura)
Bladder or urinary medications – Combining these medications with Spiriva may increase the anticholinergic effect of the drug. It may increase the chances of side effects such as dry mouth, blurred vision, constipation and difficulty passing urine.
- Darifenacin (Enablex)
- Flavoxate (Urispas)
- Oxybutynin (Ditropan, Oxytrol)
- Tolterodine (Detrol)
- Solifenacin (Vesicare)
Cough, cold, or allergy medicine – Many of these medications contain anticholinergic compounds. Using it along with Spiriva also increases the risk of side effects particularly blurred vision, constipation, dry mouth and difficulty passing urine.
- Diphenyhdramine (Benadryl, Tylenol PM)
Ulcer medications - Combining these medications with Spiriva may increase the anticholinergic effect of the drug. It may increase the chances of side effects such as dry mouth, blurred vision, constipation and difficulty passing urine.
- Glycopyrrolate (Robinul)
- Mepenzolate (Cantil)
Although drug interactions with Spiriva are unlikely to cause life-threatening adverse effects and cases of adverse drug interactions are rarely reported, it is best to avoid any possible interaction. Follow your healthcare provider’s guidelines with regards to concomitant use of these medications.
